The short answer
Choose Perifit Care ($119.20 on sale) if its standard insert (8.3 × 3.1 cm) is comfortable for you and you want to save $40. Choose Perifit Care+ ($159.20 on sale) if you want a smaller, flexible insert, or exercises in different positions, which its motion sensor makes possible. Both are FDA-cleared for the same uses, use the same free app, never need charging and come with a 100-day trial, so you can return the one that does not suit you.

Which should you choose?
Perifit Care if
- You want the lowest price for an FDA-cleared trainer with an app.
- You are happy training lying down or sitting.
Perifit Care+ if
- You find inserts uncomfortable and want the smaller, flexible version.
- You want to practise in different positions, which the motion sensor allows.
- You would like more varied, fitness-style routines to stay motivated.
Neither is cleared for anything the other is not, and we have seen no evidence that one gives better results. The research on biofeedback trainers in general is mixed; see do kegel trainers work?
Who should not use either
Perifit's "do not use" list applies to both. It includes pregnancy, the first 6 weeks after giving birth, recent surgery, a bladder, vaginal or urinary tract infection, unexplained bleeding or discharge, a history of urinary retention, a hypertonic (overactive) pelvic floor or vaginismus, moderate to severe prolapse not yet seen by a professional, and allergy to medical-grade silicone. Ask a clinician if any of these apply to you.
